Rafi Saheb's Own Song Diary, Have a look Options

As I said before that one of my patient told me that he has Rafi saheb" song s diary.I requested him to to give me for few days so than I can scan it and post it to H/F.He was very reluctant but I persuaded and convinced him to give it to me .
These are two diaries with calender of 1968 and 1969 in it,First one is numbered as
Book no.57
1-11-68
31-12-68
other one is titeled as
Book no 58
from 1-1-1969
to12-4-1969

In this diary songs are written in perticuler pattern
Most of the songs are written in very good hand writting.At the top left hand corner with the same writing pattern, is written ,Production compny name ,song NO. and films name.
While at the top right hand corner date and time is mentioned and some initials like F.T. or B.L. is mentioned.(What these initials means??)These top right hand corner entries seems to be written hurriedly and on some other occasion, than when the song was written.

ON this stanza scribling is done, at present i cannot recall whether this stanza is there in this song or not
